APL Waiver confusion.

The formatting at the bottom is screwing with me. for example:

But what about the line to the right of the "PARTICIPANT's SIGNATURE" as well. It would be easier to see in the waiver itself han just looking at my post. Do I sign both?

Line 3: Phone Number
Line 4: Street Address, City, State, Zip
Line 5: Email Address
